If you’ve recently been injured, had surgery, or received a referral for physical therapy, you may assume you’re required to go wherever your doctor or hospital tells you to go. That’s a common misconception—and it’s not true.
As a patient, you always have the legal right to choose your physical therapy provider. No matter where your referral comes from, the decision is ultimately yours.
Today, many hospitals and large healthcare systems operate their own physical therapy clinics. When a physician is affiliated with one of these systems, they may automatically recommend—or even schedule—you at their in-house therapy location.
While this can sound convenient, it’s important to understand:
Doctors may suggest a specific clinic, but they cannot require you to go there
You are not obligated to receive physical therapy within a hospital system
Your referral is valid at any licensed physical therapy clinic that accepts your insurance (or cash-based care)
In other words, a recommendation is just that—a recommendation.
Hospital systems often encourage patients to stay “in-network” because it benefits the system financially and administratively. Keeping services under one umbrella can streamline billing and referrals—but that convenience for the system doesn’t always mean it’s the best option for the patient.
This practice is sometimes referred to as provider steering. While it’s allowed for a doctor to suggest a location, patients must be informed that they have a choice.
Federal and state regulations protect patient freedom of choice. These laws are designed to ensure that healthcare decisions are made in the best interest of the patient, not based on financial incentives or corporate affiliations.
Simply put:
Your physical therapy prescription belongs to you
You can take it to any provider you trust
You do not need a new referral to switch clinics in most cases
If you ever feel pressured or told you “have to” go somewhere specific, it’s okay to ask questions—or choose a different provider.
Physical therapy isn’t one-size-fits-all. Where you go can significantly impact your recovery experience and outcomes. Many patients prefer independent or privately owned physical therapy clinics because they often offer:
One-on-one treatment with a licensed physical therapist
Longer appointment times
Greater continuity of care (seeing the same therapist each visit)
Fewer patients per therapist
A more personalized, relationship-based approach
The most important factor is finding a clinic where you feel heard, supported, and confident in your care.
If you already have a physical therapy referral:
Tell your doctor or care coordinator where you’d like to go
Ask for a copy of your prescription (or have it sent directly to your chosen clinic)
Contact the physical therapy clinic of your choice to schedule
That’s it. No extra steps. No special permission required.
You are in control of your healthcare decisions—including where you receive physical therapy. While hospitals and physicians may offer recommendations, the choice is always yours.
